who is _____ years of age and is a student at ____________________ High School, to participate in the Texas Junior Science and Humanities Symposium, January 17— 19, 2008, on the Texas A&M University campus in College Station, Texas.

We understand the students will leave their home for College Station accompanied by ___________________ (name of responsible adult) on or about January 17, 2008, and will leave College Station on or about January 19, 2008. I further understand that the students and sponsors will travel in and around College Station while the Symposium is in progress.

We understand that the Junior Science and Humanities Symposium national office has taken a broad insurance policy which covers student participants for accident and sickness and adult participants for accident benefits during the time of the Symposium and during travel to and from the Symposium.

In consideration of the benefits to be derived from participation in the Texas Junior Science and Humanities Symposium, as well as other good and valuable considerations, as father and mother and natural guardians of our (son) (daughter) _________________, I/we hereby release Texas A&M University, its officers, and employees of and from all liability for any damage which may be sustained by him/her as a result of any accident while on this trip.

I intend to be legally bound thereby.

In addition, I understand that my son's/daughter's name, photograph and school may be posted on the Internet under the OUTREACH website and also used in ANY publications OUTREACH may have.
