Forms for the 2008 Texas Science Olympiad

Please follow the instructions for each form. All forms (except event selection) are in Adobe (pdf) format.

2008 TSO Policy Form — this form is due, signed, on April 25 at the tournament check-in by the primary coach.

Liability/Release Form — this form should be filled out and signed for EVERY team member and coach that will participate in the Texas Science Olympiad. There are NO exceptions or substitutions to this form (school permission forms will NOT be accepted) — Texas Science Olympiad will not permit a team member to participate without this form!

The Release Form should be turned in at the check-in by the primary coach on April 25, 2008. Do not fax these forms in.

Event Selection Form — This on-line submission form is due by 9:00 a.m., April 14 (form will be taken off-line at 9:00 a.m. on April 14, 2007). Once you submit this form of your teams 23 selected events, you cannot change events. The online form allows for ONE submission per team only.

Trial Event Selection/Participation Form -- This on-line submission form is due by 9:00 a.m., April 14th.
Trial event(s) are optional and for fun — they do not count in the overall scoring.
